Item number 4 shows the carrying sling and tie-down strap are being checked. There should be no
signs of wear or fraying, snaps or latches are firmly in place, and metal connectors and ends are clean
and in place. Also make sure that where the sling and strap are attached to the telephone, the screws are
tight and the connectors are working.

Item number 5 requires an operational check on the telephone. It is easiest to check this with
another telephone. Make sure the buzzer works, the ring indicator lights, and you can talk and hear on
the telephone.

Item number 6 requires a check in DA Pam 25-30 to see if any modifications are due on the
telephone.

Once you have performed the maintenance on the telephone, you must then make a record of the
work you did. To do this, you need to use DA Form 2404. If you are unsure on how to fill in the
heading of the form, refer to DA Pam 738-750.
