(3) BLOCK indicates the block number on the distributing frame.
(4) PINS indicates the pin numbers used by the wires in question.
(5) DESIG indicates designation of the jack circuit involved, preferably
by jack titles on the panel.
(6) By crossing out EQUIPMENT and making suitable entries in its place,
the form can be used to indicate cross-connect jumpers between incoming and
outgoing cables, between MDFs and IDFs, and between any other circuits
appearing on the distributing frame.
