Items briefed to oncoming operators will normally be recorded in the
operator's log as they occur.
The proper preparation, utilization, and
completion of the operator's log will be detailed in the following learning
event.
Learning Event 2:
IDENTIFY ITEMS TO BE CHECKED TO ENSURE STATION LOGS AND FORMS ARE COMPLETED
AND UTILIZED PROPERLY.

The circuit log portion of DA Form 4158 is used to record mandatory
entries and incidents or conditions affecting circuit efficiency.
A new
circuit log is started at 0001 hours each day.

After completing the heading of the circuit log, the operator makes
entries in the log as events occur.
The first entry in the log is the
operator signing on duty. As with all entries, the time is entered in the
time column. The time entered is the time of day and the time zone letter.
The operator enters his personal sign (normally initials) in the operator's
sign block.
The content of the transmission or an explanation of the
particular occurrence is recorded in the remarks column. The initial entry,
operator on duty, is completed with the operator printing his rank, name,
and the words ON DUTY. Figure 4-1 shows an example of a circuit log with
the heading and first entry completed.
